I, , warrant that I am the parent or legal guardian of , a minor child (the “Child”) and have full authority to authorize this Waiver of Liability on behalf of such Child. On behalf of such Child, I understand and acknowledge that baseball is an action sport and that the natural and man-made obstacles and hazards and surface and environmental conditions may occur during the course of the game, on the field, in the stands and throughout the surrounding area, which obstacles and hazards may carry significant risk of personal injury to the Child and/or damage to Child’s property.

On behalf of such Child, I agree that such Child must take an active role in such activities to participate in Dynasty Sports Institute and accept those risks, conditions, obstacles, and hazards. Further, in consideration of the Child’s participation in the Event and on behalf of the Child, I hereby acknowledge that Child has participated in the Event at his or her own risk and has accepted the risks, conditions, obstacles, and hazards. Accordingly, I have and hereby do for myself, the Child, and our heirs, executors, administrators and assigns, waive, release and forever discharge Dynasty Sports Institute and their respective members, partners, affiliates, employees, directors, officers, agents, successors and assigns from any and all manner of actions, claims, damages, liabilities, costs, and expenses, in law or in equity, which said such Child ever had, now has or shall or may have against said Releases or which any personal representative, successors, heirs or assigns of said Child, hereafter, can shall or may have, against said Releases, for, upon, or by reason of Child’s participation in the Summer.

I authorize that Dynasty Sports Institute, and anyone authorized to use the Child’s photograph, in whole or in part, with or without any name, signature and biographical information, for any advertising or commercial purpose without any compensation being due or owing to me or Child.
